Terms of Engagement & Consent Form between Health Coach and Client


Health Coach Responsibility and Scope of Practice


Health coaching is an alliance style partnership between coach and client in an individual discussion-based process which aims to inspire the client to take steps to optimise their health & wellbeing potential. It is designed to facilitate the development of health goals, and a practical strategy or plan for achieving those goals.

Effective nutrition and lifestyle practices support the body’s natural defences and resistance to illness and disease.

 No claim or guarantee can be made about the efficacy of any advice given. Positive outcomes are also dependent upon a client’s actions, decisions and choices, and a health coach is not responsible for a client’s actions or lack thereof.

Health coach advice will be tailored to support medically diagnosed conditions and/or health concerns agreed and identified by the two parties.

Health coaches are not permitted to diagnose or claim to treat medical conditions. This is outside their scope of practice.

Health coach advice is not a substitute for professional medical advice and treatment.


Responsibilities of the Client


You are responsible for contacting your medical doctor or specialist about any health concerns you may have.

Please advise your medical doctor of the heath coach protocol you will be following, if appropriate. Also advise any other natural medicine therapist you are consulting.

It is important that you tell your health coach about any medical diagnosis you have received, or any prescription medication, natural remedies, and supplements or over the counter medication you are taking, as it may affect the coaching plan that you put together with your coach.

If you are unclear about any part of your coaching plan, then you should contact your coach immediately for clarification.

Please report any concerns about your coaching plan to your health coach for discussion at your next consultation.


Confidentiality


How we obtain, use and store your personal information, as well as how long we hold your personal information, is fully covered in our Privacy Policy which can be found on our website www.julietlawsonhealth.co.uk.


Cancellation and Re-scheduling of appointments


All appointments are booked online by the client. The client agrees that they will use their best efforts to keep all booked appointments, as they appreciate that the coach has blocked out that time in their diary specifically for their appointment.

The coach appreciates that occasionally a client may need to re-schedule. The client agrees, in absence of emergency, to provide at least 4 hours’ notice to the coach if they need to reschedule, more if they are able to, and then to reschedule as soon as reasonably practicable.

If no notice is given, and the client is a no-show for the appointment, the coach reserves the right to treat the appointment as attended, and the ability to reschedule it forfeited.
